Since upgrading from F13 to F15 I've noticed vlc no longer plays .mp4 files without issue - audio is out of sync with video and pausing and resuming playback does not restore sync, not even temporarily.

This sounds like http://trac.videolan.org/vlc/ticket/5124 so I tried to rebuild the vlc rpm with all audio related changes from vlc-1.1.git and this did indeed cure the problem. I obtained the patches with:

[vlc-1.1]$ git log 1.1.11..HEAD --oneline modules/audio_output

as a first cut. Obviously not all of these patches affect sync, some don't even affect PA. Would it be possible to backport (some of) these patches to vlc-1.1.11-1.fc15.rpm?

Can you give a try on vlc-1.1.12 using:
yum update vlc --enablerepo=rpmfusion-free-updates-testing
